Synopsis

The sixth edition (2000) is recommended for a minimum core collection by Brandon/Hill. The two volumes of the 7th contain 66 chapters by kidney specialists--most from the US, but quite a few from countries including Israel, France, Lebanon, The Netherlands, Canada, Brazil and the Czech Republic. The first 17 chapters in volume one address the elements of normal renal structure and function. The remaining chapters cover disturbances in control of body fluid volume, pathogenesis and pathophysiology of renal disease, and management of patients with renal failure. Topics include nutritional therapy in renal disease; transplantation immunobiology; cardiovascular aspects of chronic kidney disease; renal neoplasia, laboratory assessment of kidney disease; adaptation to nephron loss; effects of aging on renal function and disease; and specific pharmacologic approaches to clinical renoprotection. This fifth edition has seven new chapters, 12 completely rewritten chapters, and 41 chapters extensively revised and updated since the fourth edition was published in 1991. Each chapter has been written by authors considered to be experts in their respective fields and who have made substantial contributions to our understanding of these areas of nephrology. This two-volume set provides a comprehensive source of information containing state-of-the-art discussions of the basic physiology and pathophysiology of the kidney. This book has already been established as a classic in nephrology and serves as an excellent reference source for anyone in the field, ranging from the student to the clinical nephrologist to those in the related fields of pediatrics and urology. The current edition has more than 12,000 illustrations and tables and approximately 35,000 references that have been published through 1995. It is well organized and continues to be divided into five major headings, with each chapter being organized into subheadings to improve readability as has been done successfully in previous editions. This is a high-quality, readable text for anyone seeking information related to the normal physiology, pathophysiology, clinical features, and management of diseases of the kidney. In addition, this edition contains new chapters ranging from cellular to gerontologic aspects of renal function. These updates certainly make purchasing the fifth edition worthwhile. This book is useful for individual purchase and for a reference in the library. It should be made available in academic bookstores.

From The Critics

Reviewer: Jon Webb, MD(University of Kentucky College of Medicine)
Description: This eighth edition of a classic textbook in nephrology provides a comprehensive review of the pathogenesis, pathophysiology, and clinical decisions regarding the kidney.
Purpose: The goal is to provide a reference on the physiology of the kidney with a focus on the development of pathology and subsequent clinical management. In its attempt to focus on modern literature, the book is shorter than previous versions. This edition stresses the importance of incorporating new information. First, 40 percent of the contributors are new to this edition and, second, the authors were encouraged to focus on literature published after 1990. Lastly, an e-dition is incorporated, providing electronic access to chapters, tables, and relevant journal articles.
Audience: This is an excellent tool for nephrologists, both clinical and research oriented. The masterful organization and colorful display will also attract medical students, internists, pediatricians, and urologists, providing an easily accessible resource for specific topics on kidney disease.
Features: "This edition is newly organized in 12 sections that cover normal renal function (nine chapters on embryology, anatomy, and physiology of tubule functions); integrated control of body fluid volume and composition (seven chapters on molecules that influence renal function followed by a review of electrolyte disorders); epidemiology and risk factors in kidney disease (a new section of five chapters focusing on aging, gender, and other factors that affect renal disease risk); pathogenesis of renal disease (seven chapters describe the diagnostic assessment of kidney disease and include an extensive imaging library); disorders of kidney function (authors with expertise in their respective areas contribute 10 chapters on known major clinical entities); genetic basis of kidney disease (three chapters with an emphasis on cystic diseases and tubule transport disorders); hypertension and the kidney (five chapters on both primary and secondary hypertension, with a breadth of information on antihypertensive drugs and diuretics); the consequences of renal failure (new research findings are a highlight of these six chapters on the systemic ramifications of the uremic state); conservative and pharmacological management of kidney disease (five chapters review the use of common medications in chronic kidney disease and update precautionary measures necessary in advanced disease); invasive therapy of renal failure (five chapters address treatment modalities including hemodialysis, peritoneal dialysis, and plasmapheresis and provide an overview of critical care nephrology); renal transplantation (three chapters review the basic immunology of transplantation and provide a comprehensive discussion of the process of donating and receiving a kidney); frontiers in nephrology (the second new section presents five chapters on the exciting possibilities of the future of managing kidney disease, including xenotransplantation and stem cell utilization). Fourteen new chapters have been added to this edition, including interventional nephrology and endocrine aspects of kidney disease, but the total number of chapters has increased by only four. The dynamic visual improvements provide much enhanced organization over previous editions. In addition, the book features more than 550 color high-quality photographs, an abundance of figures, tables, and algorithms, all accessible thru the electronic e-dition; an image library for presentations; and the constantly evolving electronic version to keep readers informed of the latest trends in management of kidney disease. "
Assessment: This is an impressive update from the previous edition, providing a new colorful presentation with a focus on the constantly evolving trends in kidney disease. The sections are well organized, and the chapters are presented in a logical manner. The figures, tables, and images enhance the discussion of each topic. I particularly enjoyed the focus on upcoming trends and potential therapeutic options in the new section on frontiers in nephrology. This remains the gold standard in nephrology textbooks, and I highly recommend it to anyone searching for a resource on kidney disease.

From the Publisher

“Each chapter has been written by leaders in the field, with the authorship remaining mainly North American and European in origin. There s a consistent chapter structure throughout the two volumes with easily readable text, and appropriate use of subheadings, figures and black and white plates. There is an extensive bibliography at the end of each chapter.

The text remains comprehensive in this discussion of the range of renal disease, although discussion of the management of renal failure remains relatively brief as necessitated by the need to keep the text to a manageable size. One of the strengths of the current edition is the discussion of management options in various clinical settings.

Overall, the current edition of ‘The Kidney’ cements its position as one of the pre-eminent textbooks of nephrology. It remain an invaluable text for nephrology clinicians, trainees and scientists.”

Reviewed by www.nephrology.edu.au – Nephrology Newsletter

“The general presentation of the textbook is outstanding, with an immediate outline of chapter subtitles at the top of each chapter, a diligent choice of only two colours throughout, namely black and blue (except for three colour plates at the beginning of volume 2) and numerous, high quality illustrations of all kinds.

It deserves a permanent place on the book shelves of all clionical scientists, basic scientists and students who are interested in the broad field of nephrology.”
