Overview
Spinal manipulation under anesthesia (MUA) is a procedure intended for patients who suffer from musculoskeletal disorders in conjunction with biomechanical dysfunction. Performed using monitored anesthesia care, this technique is overcoming its controversial image and receiving regular use by a great number of practitioners.
Manipulation Under Anesthesia is the first book to present the procedure from a historical perspective to present day use. Focusing on all aspects of MUA technique, the text provides an outline that represents this procedure from a scientific perspective, along with information concerning patient selection criteria, and documentation explaining outcomes from patient procedures. It discusses the protocols and standards by which the procedure should be incorporated into the facilities where it is being performed, features an overview of the generic procedure and policies for documenting results, and addresses the overall use of the procedure from pre-MUA through post-MUA rehabilitation.
With justification for the procedure from both a clinical and patient management standpoint, this unique compilation of information is useful as both a reference and as an educational aid for doctors. It provides the necessary tools for investigating the procedure and incorporating it into any healthcare specialty.
Editorials
From The Critics
Reviewer: Dana J Lawrence, DC, MMedEd, MA(Palmer College of Chiropractic)Description: This book is designed to present information covering all aspects of manipulation under anesthesia. It covers the science underlying the procedure, patient selection procedures, documenting outcomes, actual patient procedures and legal and ethical issues governing the practice. Its focus is on pain relief, and it also covers post-MUA rehabilitation techniques and procedures.
Purpose: The purpose of this book is to provide comprehensive information about the use of manipulation under anesthesia as a means of pain relief. To my knowledge there has never been a book covering this topic, though the procedure has been taught as part of several chiropractic college continuing education programs. Given the growing interest in this procedure, the objectives are indeed worthy ones, and in large measure this book does meet the goals and objectives set for it by the author.
Audience: "This book is largely written for practicing chiropractors, though it will be of interest to other manipulators: medical physiatrists, osteopaths, naturopaths. At present MUA is not part of the undergraduate chiropractic curriculum, but is offered as a continuing education program, where this book will be highly needed and coveted. I do not see it being of interest to students, but I think there is a market for it among field practitioners. The author has been one of the leading practitioners of MUA within the chiropractic profession for many years. He is indeed seen as an authority on the subject. "
Features: The book is designed to provide comprehensive information about manipulation under anesthesia. To that end, it covers nearly everything about the procedure, from its historical underpinnings and its scientific basis, to how pain is modulated and tissue repaired, and to when and where not it should be used. The author spends time looking at literature both in favor of and against MUA, and fairly presents the debate. Much time is spent on examination and treatment, and the treatment material is amply supported by photographic illustrations which demonstrate the specifics of many techniques. There is also a section that looks at the business aspects of MUA, from how to gain hospital privileges to proper recordkeeping, and there is information on anesthesia as well as rehabilitation. Finally, sample forms are provided.
Assessment: First, a disclosure: I am one of many people acknowledged in the front matter for the book, as I was formerly the postgraduate dean at National University of Health Sciences, which offered MUA as a CE course offering. This book is a culmination of many years of work for Dr. Gordon, who has labored often in obscurity to build the practice of MUA within the chiropractic profession. He has drawn into the book other leading practitioners of MUA, such as Dr. Robert Francis, and this helps offer greater strength and rigor to the book. It covers virtually all aspects of MUA, and to that end is as definitive a statement as currently exists within the chiropractic profession. I find the use of algorithms helpful, and the section on patient selection also quite useful. A great deal of time went into this, and it shows. I recommend it.
