Overview
Because physicians generally have neither the time nor inclination to perform mathematical calculations to code their medical documentation, the author finds that only 15 to 25% of E/M medical records are submitted correctly. Levinson, the CMO of iMedX, attempts to provide "tools, not rules," such as NPP ("Nature of the Presenting Problem") that allow doctors to integrate documentation and coding with the process of providing care.
